Other meta-analyses that have included observational studies with less stringent inclusion criteria have demonstrated variable improvements in fasting glucose, insulin resistance, and HbA1c levels.138, 325, 326 Given the link between LTBF and morbidity and mortality in older men, evaluating bone density is an important step in the assessment of patients with testosterone deficiency. In trials, patients with low testosterone have demonstrated statistically significant improvements in erectile function, anemia, BMD, lean body mass, and depressive symptoms. If the Hct exceeds 50%, clinicians should consider withholding testosterone therapy until the etiology of the high Hct is explained.187 While on testosterone therapy, a Hct ≥54% warrants intervention. Another multi-center study compared the effectiveness and risks of transdermal and IM testosterone in 66 men aged years old. After 180 days of treatment, only 1 patient in the 50mg gel arm, 3 patients in the 100mg gel arm, and no patients in the testosterone patch arm were found to have gynecomastia. Hypergonadotropic hypogonadism, which is not a contraindication to begin testosterone therapy, can result from a number of conditions, including congenital abnormalities (KS being the most common), iatrogenic causes (e.g., bilateral orchiectomy, testicular radiation, chemotherapy), testicular trauma, infection, or autoimmune damage. They will also do a physical exam and ask whether you have the symptoms of low T before prescribing anything. Taking the drug is often a lifelong commitment (unless the low T is caused by a medical illness), so you'll need to be continually monitored by a doctor. You need a prescription from your doctor to access testosterone. An intranasal testosterone gel applied topically into the nose was approved by the FDA in 2014. The progressive hydration tablet with a matrix containing 30 mg of testosterone is placed in position on the gum above the right or left canine and is held in position for approximately 30 seconds. The most common adverse effect with patches is application site reactions, which have been historically reported in up to 60% of patients.181 Other adverse effects include pruritus, application site vesicles, and back pain.431 Compared to topical gels and solutions, the rate of transference is likely minimal. Testosterone patches consist of a mixture of testosterone, penetration agents, and a gelatinous matrix separated from the skin by a microporous membrane. Adverse effects specific to topical preparations include application site reactions (3-16% erythema or rash), and risk of transference. Liquids and gels should be applied to clean, dry skin, and the treatment site should not be washed until the time of next application to optimize delivery. In a prospective study done by Katz et al., ADAM scores improved in all but one question (i.e., change in height) (18). Developed by Morley in 2000, recent studies have demonstrated a lower sensitivity and specificity than originally estimated (17), but the questionnaire is nonetheless used widely in clinical practice. Mean testosterone values over a 7-day time period were 1,659, 896, and 422 ng/dL for IM testosterone SQ 100, and SQ 50, respectively. The half-life for IM testosterone was also shorter at 173 hours versus 240 hours for SQ testosterone. Injectable testosterone is available in several forms, including short acting and long-acting preparations. The mean testosterone concentration was 421 ng/dL.436 In a 90-day open label trial of 306 testosterone deficient men using two actuations (11mg) of the drug applied three times daily, results were reported for 73 men at day 90. The product is provided in a metered pump that supplies 5.5 mg of testosterone per actuation. The symptoms of low T are relatively non-specific and can be seen with several other medical conditions, and an accurate diagnosis is vital to direct treatment. On PubMed, 24.8% of supplements had data showing an increase in T with supplementation, 10.1% had data showing a decrease in T, and 18.3% had data showing no change in T. For example, some studies suggest that TRT decreases your risk of heart attack over time, while others say it increases your risk. If you stop taking testosterone, your levels will return to baseline.
